SUMMONS ONLINE
EXCUSE AFFIDAVIT
If
you claim an exemption because of illness to yourself or
illness to another who would require your attendance,
please complete the online affidavit below and e-mail the doctor’s excuse to the Court Administrator
immediately. You
will receive an e-mail confirmation as to whether you
are excused.
When the excuse is claimed because your
attendance is required due to illness of another, every
effort should be made to have someone else attend the
person so that you can serve on jury duty.
If
your attendance would cause serious financial loss to
yourself or your business, please e-mail the affidavit
explaining this to the Court Administrator immediately.
It will not be necessary for you to call the
Court Administrator or the Clerk.
You will receive an e-mail as to whether you are
excused. If
you are not notified that you are excused, then you are
expected to appear at the time indicated on your summons
or as per recorded instructions.
If
you are over 65 years of age or have served on the jury
panel in the Circuit Court in the actual trial within
two (2) years and do not wish to serve, so state on the
affidavit and e-mail the Court Administrator
immediately. It
will not be necessary that you appear for duty.
You may serve if you desire.
If there is a deficiency of jurors, you may be
notified to appear.
If
you claim any other excuse because of emergency or
hardship to yourself, please state your reasons on the
affidavit and e-mail the Court Administrator
immediately. You
will receive an e-mail as to whether you are excused.
If you are not notified that you are excused,
then you are expected to appear at the time indicated on
your summons or as per recorded instructions.
Names
are randomly selected from the Warren County voter
rolls. If
you are no longer a resident of Warren County and have
received a jury summons, you will need to contact the
Circuit Clerk’s office to have your name removed from
the voter roll. Also, please complete the affidavit and
e-mail it to the Court Administrator immediately. You do
not need to appear for duty.
